Instructions:
Fill out the fields and press the 'build item' button.
Don't worry about getting it perfect, you can edit it later.
Copy the selected text to your clipboard, and paste that text into the desired place (use the 'Paste from Word' tool) into the article you are editing in the Chorus editor, in the normal (visual) view.
Edit the item and add links as needed. Publish normally.
